This woman in her mid to late 30s came to me dissatisfied with her best appearance. She felt they were "rocks in the sock". She had previously undergone a saline breast augmentation above the muscle and she had the classic signs of such-a vertical drop from her collarbone down to where her breasts "start" with associated unattractive concavity and emptiness. She was also self-conscious of the "rippling". After extensive discussion with the patient, she opted for larger sized implants. We both agreed to place the implants "under the muscle" (pectoralis major muscle) in order to give a smoother, gradual transition from the collarbone to the nipple. Switching the saline implant, which is associated with more rippling, to a Mentor "Memory Gel" silicone implant gave the patient more of a natural look and feel to her breasts. Sometimes their breasts make the patient feel "droopy", especially after they've had children and have nursed. Many times, the patient feels like they need a breast lift (mastopexy) with its attendant scars, but, commonly this is not necessary.
