This patient had undergone a previous breast augmentation in the past, prepectoral or above the muscle, so to speak. Unfortunately, though she attained the size she was looking for, she was not happy with the shape of her breast. She came to one of the top breast implant revision specialists, Dr. Randal Haworth of Beverly Hills to seek advice and surgical improvement. After a thorough examination, Dr. HAWORTH felt it best as an expert breast implant surgeon to exchange her implants from "above the muscle" to subpectoral or "under the muscle". He also diagnosed she had implant ptosis or, in layman's terms, her implants had fallen too inferiorly or downwards, below the inframammary crease.
Consequently, he decided to also incorporate a capsullorraphy, or re-creation of her inframammary crease to a higher, more natural position.

When the implant is falling too far down below its optimal position, the breast looks longer while the nipples and areolae are tilted too far upwards, even above the natural confines of a brassiere. By performing a capsulorrhaphy, the implants sit on a higher, more natural fold position and consequently have a beneficial effect upon the appearance of the breasts themselves.
