This man sustained trauma to his nose many years ago and during an attempted repair sustained a chronic infection of unknown etiology. This unfortunately led to destruction of his septal cartilage and a subsequent "saddle nose" deformity (similar to a boxer's nose). This is best visualized from his profile view. After a number of unsuccessful surgical interventions, he came to me to perform a complex nasoseptal reconstruction or revision rhinoplasty. As a Beverly Hills specialist in revision rhinoplasty, I immediately realized that his nasal bridge would need good solid support as afforded by a rib graft. This is one of the few occasions where ribs grafts are a necessity.

During this case, the cause of his recurrent infections was discovered to be an epidermal inclusion cyst that occasionally flared up in debilitating infections. The cyst most probably arose from improper surgical closure during his previous rhinoplasty repairs. Due to his severe deformity, a rib graft was harvested to complete the complex revision rhinoplasty and carefully tailored to size and fit to both provide support and shape to his nose. Of course, this rib framework was assembled in such a way to balance his other masculine facial features. Fine reinforcing wires were placed within each rib graft to prevent warping, an otherwise fairly common problem with rib cartilage in the nose.
