NOTICE OF PRIVACY PRACTICES

To our patients. This notice describes how health information about you (as a patient of this practice) may be used and disclosed, and how you can get access to your health information. This is required by the privacy regulations created as a result of the health insurance Portability and Accountability act of 1996 (HIPAA). Our office reserves the right to change this privacy policy without advanced notice in the future. As well, any changes or updates will be posted in the revision of this document and on our site.
OUR COMMITMENT TO YOUR PRIVACY
Our practice is dedicated to maintaining the privacy of your health information. We are required by law to maintain the confidentiality of your health information. By using our site, you consent to our privacy policy. We realize that these laws are complicated, but we must provide you with the following important information.
USE AND DISCLOSURE OF YOUR HEALTH INFORMATION IN SPECIAL CIRCUMSTANCES
The following circumstances may require us to use or disclose your health information:
  • To public health authorities and health oversight agencies that are authorized by law to collect information.
  • Lawsuits and similar proceedings in response to a court or administrative order.
  • If required to do so by law enforcement official.
  • When necessary to reduce or prevent a serious threat to your health and safety or the health and safety or the health and safety of another individual or the public. We will only make disclosures to a person or organization able to help prevent the threat.
  • If you are a member of U.S. or foreign military forces (including veterans) and if required by the appropriate authorities.
  • To federal officials for intelligence and national security activities authorized by law.

  • For Workers Compensation and similar programs.
INFORMATION DISCLOSED TO OUTSIDE PARTIES.
We do not share, sell, rent, lease, trade, or otherwise transfer to outside parties your personally identifiable information. As needed we work with trusted third parties who assist us in conducting our business and relevant information is exchanged to accomplish servicing your needs. Non-personally identifiable web site visitor information may be provided to other parties for marketing, advertising, or other uses.
YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION
Communications. You can request that our practice communicate with you about your health and related issues in a particular manner or at a certain location. For instance, you may ask that we contact you at home, rather than work. We will accommodate reasonable requests. You can request a restriction in our use or disclosure of your health information for treatment, payment, or health care operations. Additionally, you have the right to request that we restrict our disclosure of your health information to only certain individuals involved in your care or the payment for your care, such as family members and friends. We are not required to agree to your request; however if we do agree, we are bound by our agreement except when otherwise required by law, in emergencies, or when the information is necessary to treat you. You have the right to inspect and obtain a copy of the health information that may be used to make decisions about you, including patient medical record and billing records, but not including psychotherapy notes. Roxbury Dr., Suite 901, Beverly Hills, CA 90210 You may ask to amend your health information if you believe it is incorrect or incomplete, and as long as the information is kept by or for our practice. To request an amendment your request must be made in writing and submitted Dr Randal Haworth M.D., F.A.C.S. 465 N. Roxbury Dr., Suite 901, Beverly Hills, CA 90210 You must provide us with a reason that supports your request for amendment. Right to a copy of this notice. You are entitled to receive a copy of this Notice of Privacy Practices. You may ask us to give you a copy of this notice at any time. To obtain a copy of this notice, contact our front desk office. Right to file a complaint. If you believe your privacy rights have been violated, you may file a complaint with our practice or with the Secretary of the Department of Health and Human Services. To file a complaint with our practice, contact Office Manager, The Haworth Institute, 465 N. Roxbury Dr., Suite 901, Beverly Hills, CA. 90210 all complaints must be submitted in writing. You will not be penalized for filing a complaint. Right to provide an authorization for other uses and disclosures. Our practice will obtain your written authorization for uses and disclosures that are not identified by this notice or permitted by applicable law. Occasionally, at our discretion, we may include or offer third party products or services on our website. These third parties and their web sites have separate and independent privacy policies. We have no responsibility or liability for the content and activities of these, or linked sites. Nonetheless, we seek to protect the integrity of our site and welcome any feedback about our information or any of these sites.
CALIFORNIA ONLINE PRIVACY PROTECTION ACT COMPLIANCE
Because we value your privacy we have taken the necessary precautions to be in compliance with the California Online Privacy Protection Act. We therefore will not distribute your personal information to outside parties without your consent.
CHILDREN’S ONLINE PRIVACY PROTECTION ACT COMPLIANCE
We are in compliance with the requirements of COPPA (Children’s Online Privacy Protection Act), we do not collect any online information from anyone under 13 years of age. Our website, and online products and services are all directed to people who are at least 13 years old or older. You control what kinds of information you receive from us. In order to receive information about services, updates on health privacy, and legislative action you must opt-in to our e-mail list. We will not automatically send your information without your consent. Only your e-mail address is required. You have the option to provide your name and mailing address. You may opt out of any kind of e-mails or all e-mails entirely. (Note: if you make a donation or send a letter to a decision maker, you will receive a confirmation e-mail but are not automatically put in our e-mail group.) Most e-mails also include a link to unsubscribe out of e-mail at the bottom of the message. You can also e-mail us directly at [email protected].
WE DO NOT COLLECT DATA
We do not use Permanent Cookies or “remember me” features. This means you have to personally enter any of your information every time. The “session based” or Temporary Cookies on our site will only store activity for a current session to allow you a smoother user experience. These temporary cookies are discarded as soon as you close your browser. They are not collected or stored in any way. We do not use or analyze IP addresses or web logs. Permanent web log activity is protected using secure data procedures accessible only by password-protected entry.
